Q. I have a crooked smile and would like it fixed. My lips are full, so I do not want filler. Will I need surgery to fix it?

A. No indeed, filler is only good for asymmetry correction for those also wishing fuller lips. Surgery is very risky in this area. Sometimes the problem is the teeth and for that a cosmetic dentist is the best option. For most patients, however, very carefully placed Botox can improve an asymmetrical smile. The key is to know exactly where to place the right number of units. I recommend a board certified dermatologist who does a lot of this procedure.

Botox for Gummy Smile, Before and After Q. I do not want big lips, but my gums show when I smile and I do not like the way I look. It even keeps me from smiling or I find myself covering my mouth when I laugh at something really funny. Is surgery the only option to help me, since I do not want big fish lips from filler injections ?

A. First, neither surgery nor filler injections into your lips is the optimal solution for your “gummy smile”. While I agree, fillers are not the answer, please remember to look at my gallery to see lip results. Good injectors do NOT give patients fish lips. Artistic technique from an experienced, expert, board certified injector can give lovely enhancement of the lips. No, neither surgery nor fillers is the best treatment for the gummy smile. The use of strategically placed Botox (yes! Botox!) is the optimal procedure to improve the gummy smile.

Q. I have heard Botox being used for jowls? How does that work?


A. The platysma muscle pulls the face downward. Microinjections of Botox along the jaw line and upper neck help neutralize the downward pull and help define and lift drooping jowls. In addition, the depressor anguloris oris is a muscle that pulls the mouth corner down and accentuates the jowls. The mentalis muscle pulls the mid chin up and makes the jowls look worse, so by injecting both of these muscles, the jowls get lifted. These Botox injections can complement other procedures such as Exilis or Ulthera skin tightening, or filler injections along the jaw line for further lift.
